Comprehensive Performance Evaluation Request Form

The Comprehensive Performance Evaluation (CPE) Request form must be completed and submitted when an individual filter has confirmed effluent turbidity readings above 2.0 NTU on two separate occasions during any consecutive two months. The SWMOR spreadsheet automatically determines if a CPE is needed.
