Filter Assessment Report Forms and Instructions
The Filter Assessment Report (FAR) must be completed and submitted when an individual filter has confirmed effluent turbidity readings above 1.0 NTU on three separate occasions during any consecutive three months. The SWMOR spreadsheet automatically determines if a FAR is needed. This file contains the FAR form, an example of a completed FAR form, and instructions.
