Computer-Based Testing (CBT) Application Request Form

Before you register for a computer-based licensing exam, you must have an approved application. To request an application, complete this form:


Enter your first and last name
Mailing Address Line 1 should include Apartment & Suite Numbers
Licensing Application(s) Being Requested ■
Select the program(s) for which you are requesting an application.

Remember: Don’t register for your exam until you have received your approval letter.

If you fail to get approval before registering for your CBT exam:

  • The system will not allow you to access the exam.
  • You might have to forfeit your fee with the CBT center.
