PLEASE NOTE: Effective September 1, 2017, applicants must submit the NOI, NEC, NOT, and NOC forms using the ePermits online system available through the TCEQ website or request and obtain an electronic reporting waiver. Electronic reporting waivers are not transferrable and expire on the same date as the authorization to discharge. Learn more about submitting electronic forms for stormwater general permit here.

Paper Forms:

If you do not have access to a computer, please call the TCEQ Stormwater Processing Center at 512-239-3700, to obtain the paper application forms.

Hazardous Metals Waiver Form : This form is required to be completed and kept on site at regulated facilities that can certify that the hazardous metals listed on this waiver meets at least one of the three criteria found in Part III.D.1.(e) (i-iii) for the discharge location/respective outfall that is authorized by the TPDES Multi-Sector General Permit.

The following benchmark monitoring form may be used for record keeping in the Stormwater Pollution Prevention Plan (SWP3). The results must be submitted online using the NetDMR reporting system unless the permittee requests and obtains an electronic reporting waiver. Benchmark Monitoring Form : This form is for recording Semi-annual benchmark sampling, which must be conducted at least once during the first full monitoring period (January through June or July through December) following permit issuance, and then once during each monitoring period for the term of the general permit.
