Filter Reports for Individual Media Filters
If an individual media filter produces water with elevated turbidity that suggests declining performance, inadequate filtration; or, if the performance cannot be determined, additional investigative steps must be taken by the public water system. These requirements do not apply to membrane and cartridge filters, or to water treatment plants with an active Corrective Action Plan issued by the TCEQ. Once data is entered into the Surface Water Monthly Operating Report (SWMOR) spreadsheet it will automatically determine if a Filter Profile Report, Filter Assessment Report, or Comprehensive Performance Evaluation is needed.
Use the information below to determine the type of investigation report to submit.
Filter Profile Report (FPR)
When do you need to submit an FPR?
- If a single filter produces water with consecutive, 15-minute turbidity readings that are 1.05 NTU or higher.
- Complete an FPR within 7 days of the exceedance and submit the report with your SWMOR. The SWMOR will prompt you to complete a report once data has been entered.

Filter Assessment Report (FAR)
When do do you need to submit a FAR?
- If a single filter produces water with consecutive, 15-minute turbidity readings that are 1.05 NTU or higher, three or more times during a consecutive three-month period.
- Complete a FAR within 14 days of the third exceedance and submit the report with your SWMOR. The SWMOR will prompt you to complete this report once data has been entered.

Comprehensive Performance Evaluation (CPE)
When do you need to submit a CPE?
- If a single filter, or combination of filters, produces water with consecutive, 15-minute turbidity readings that are 2.05 NTU or higher, on two separate occasions during two consecutive months.
- Submit a CPE form immediately. The SWMOR will prompt you to complete this report once data has been entered, but do not wait until the end of the month to submit this request form.
