Forms and documents for use by the partners in the Clean Rivers Program for managing and executing of their grant agreements.
Planning Agencies can obtain any of these shell documents from their CRP Planning Agency Contract or from their TCEQ Project Manager on the CRP team.
Reimbursement Requests
- Financial Status Report, with Supplemental Forms
- Found in Attachment A of the Planning Agency's Contract.
- Submitted quarterly or in accordance with contract terms.
- Budget Revision Request Form
- Found in Attachment C of the Planning Agency's Contract.
- Submitted when the Planning Agency needs to move funds between budget categories.
- Historically Underutilized Business (HUB) Form
Administrative Forms and Information
- Contractor Evaluation Form and Category Descriptions
- Found in Attachment E of the Planning Agency's Contract.
- Submitted at the end of each fiscal year.
- Release of Claims Form
- Found in Attachment B of the Planning Agency's Contract.
- Submitted with the final invoice of the contract term.
